Step 1:
Call toll free 866-695-VEIN (8346) or complete our Request A Consultation Form.

Step 2:
We ask that you take the time to complete some information online about yourself, in order for us to provide safest care to you. It is important to provide your email so we can send you a link to our new patient forms online. Please complete the online forms before your appointment.

Step 3:
Please bring the following items with you to your appointment:

1. Health Insurance card
2. Photo ID
3. Primary Care Physician Referral (if required by your insurance plan). Please make sure you obtain this before your appointment.
4. Payment method. For co-payments, we accept personal checks, Visa, MasterCard, Discover, Care Credit and cash.

If you prefer not to provide an email address, download, print and complete the following PDF documents. Bring the completed forms to your first appointment. Also, please bring the list of items noted in Step 3.

If you do not have the ability to complete the online forms or print the ones provided here, please arrive 15 minutes prior to your appointment time to complete in our office.
